The Clinic At Sagewood - Located within the HealthyLife™ Services Clubhouse for the times when you are not feeling well, for general wellness check-ups, and access to a variety of health services. Sagewood offers regular clinic hours for both urgent illness services and routine primary care. To ensure that appropriate clinical protocols are engrained within the foundation of the clinic operation, the physician who has been assigned as the Medical Director of Acacia Health Center will also provide oversight for the nurse practitioners in the clinic.

A Health Care Navigator is also available to every resident of Sagewood to provide assistance in securing the appropriate level of health care whether provided in your residence by qualified and approved home health care givers, in the HealthyLife Services outpatient nurse practitioner clinic or in Acacia Health Center's assisted living, memory care or skilled nursing care suites.
